Erie Family Health Center collects personal information only with your explicit consent via sign up forms, questionnaires, event registrations and donation forms. Erie uses a third-party provider, Constant Contact, to manage your contact information. Constant Contact provides users the opportunity to opt-out of receiving e-news from Erie through a special online form. You may choose to receive only specific communications or none at all. You may also update your contact information previously provided to us through Erie’s online form, which is managed by Erie staff.
