CountyCare is available now and can pay for many of your health care services.

CountyCare is a new Cook County Medicaid program for adults. It may pay for most of the services you receive at Erie. And it also covers other medical services you may receive outside of Erie such as hospital visits, medical equipment, home health services and physical therapy.

You, your friends and other family members may be eligible for this exciting program!

To qualify for CountyCare patients must:

  • Have annual income at or below $15,282 for individuals and $20,628 for couples
  • Live in Cook County
  • Be 19-64 years old
  • Not be eligible for other Medicaid plans
  • Not be eligible for Medicare
  • Be a legal immigrant for five years or more or a United States citizen
  • Have a Social Security number or have applied for one

Erie specialists will help you enroll in CountyCare.

You may enroll in CountyCare at any Erie location. It takes about 25 minutes and our trained specialists will help you every step of the way. You will need to bring certain documents verifying your identity, income and other information to complete your enrollment. Click here for details.
