Students who do not have proof of this exam by early October will not be able to attend school.

• Children entering kindergarten, 6th أو 9th grade don’t need a new school physical exam if they had a COMPLETE well-child visit after October 2023.
• If you think your child had a complete well-child visit and just needs the school physical form filled out, message your provider in Erie MyChart, or call 312-666-3494 and leave a message with your child’s name, date of birth, and phone number.  We will review the request and either send the completed form within 10 business days or contact you to schedule an in-person visit if one is needed.
• When you call us, please let us know if your child also needs an exam for their sports activities.  That may require an in-person visit, even if he/she had a physical since October.
