AmeriCorps School-Based Health Educator with Erie Family Health Centers 

In this role you will provide students with vital health information to empower students to live healthy lives and to advocate for themselves in the healthcare system. From you, students will also learn about healthcare services available to them at Erie and how to more easily access those services. You will

  • teach health education curriculums to students in schools and community organizations
  • host health promotions activities at school events and during school lunch
  • develop monthly teen health topics for school bulletin boards and newsletters
  • coordinate 1:1 nutrition and health coaching meetings
  • connect students to Vision specialists and Dental services
  • support recruitment and engagement of Erie's School-Based Advisory Board and Youth Advisory Committee
  • facilitate other projects collaborating with Erie staff to decrease barriers to student access to health information and services and increase student engagement in Erie health initiatives

AmeriCorps Lending Hands for Life (LHL) Educator with Erie Family Health Centers

In this role you will work closely with Erie's LHL Team to serve HIV-positive patients, patients with Hepatitis C (Hep C), and patients at higher risk for HIV, implementing patient education and departmental improvement initiatives. You will

  • develop quarterly health topics to present on LHL Program bulletin boards
  • help plan LHL Healthcare Awareness Days, including planning and executing agency-wide World AIDS Day Programming
  • create educational handouts for LHL program patients and staff, including PrEP program materials for use by providers, Patient Navigators, and patients
  • collaborate with the team to coordinate and facilitate the LHL Consumer Advisory Council (CAC), as well as three monthly Patient Support Groups for HIV+ patients, and the monthly PrEP Patient Support Group
  • assist with Team Erie's participation in the AIDS Run/Walk
  • conduct quality improvement and evaluation measures on patient health outcomes related to screening completion rates, access to care, and health outcomes for HIV, PrEP and Hep C programs
  • support with registry of Erie’s patients with Hep C
  • alongside the LHL Coordinator, provide support with patient PrEP care, including intakes, appointment follow-ups, reminder calls, and no-show follow-up; and provide support with HIV and Hep C intakes
  • enroll patients in ADAP and pharmaceutical manufacturer assistance programs for HIV+, Hep C+, and high-risk HIV- patients
  • administer Rapid HIV tests - simple finger prick method and conduct pre and post-test counseling with walk-in patients and community members

AmeriCorps Social Drivers of Health (SDOH) Educator with Erie Family Health Centers

In this role, you will

  • screen clients for needs (related to social drivers of health) either via telephone or in-clinic conversations, and connect patients to Erie and community resources (such as emergency financial assistance, transportation, food assistance, rental and utility assistance, food assistance, medical transportation, and more)
  • enroll patients in services, benefits, and programs for which they are eligible (pharmaceutical programs, LINK, financial applications for hospital referrals, early intervention support, DME, urgent referrals and more)
  • help patients schedule medical appointments
  • help patients with digital navigation, including Erie MyChart enrollment, setting up telehealth capabilities, and other digital assistance related to health
  • educate and empower patients to navigate future SDOH needs
  • complete data entry and reporting requirements for SDOH
  • assist with SDOH patient registry, and assist with managing SDOH resources

AmeriCorps Health Promotions Educator with Erie Family Health Centers

This position is for someone with deep commitment to providing compassionate care that honors the individuality of every patient. An ideal candidate

  • enjoys patient interaction and collaborating with program leaders to coordinate services/programs
  • has strong communication skills to effectively communicate with patients, healthcare providers, and team members
  • enjoys working as part of a collaborating team to achieve high levels of productivity and maximize services
  • has flexibility to adapt to change, and is able to independently handle patient care responsibilities and other work as assigned

In this role you will:

  • promote healthy lifestyle modification
  • support productivity and health outcome targets for department-wide disease management activities
  • promote, recruit for, and facilitate group education sessions (including recruiting a minimum of 10 appropriate patients per month for different groups)
  • help recruit for, coordinate logistics for, and, when needed, facilitate Erie's Diabetes Prevention Program
  • complete disease management assessments
  • deliver self-management health education to patients at in-person and telehealth visits
  • assess patients' educational needs and barriers, and in consultation with the patient's care team, perform brief health education interventions using approved curriculum
  • conduct a minimum of 85 - 1:1 health education visits per month
  • conduct patient outreach and follow-up
  • schedule follow-up medical appointments, and refer patients to appropriate patient program services
  • participate in approved community outreach and education events
