Erie MyChart Patient Portal

If you are an Erie patient, ask at the front desk or call 312.666.3494 for a link to set up your Erie MyChart patient portal account. Then, through the MyChart app or at  myeriehealth.com you can log in to your Erie MyChart account to message your care team, see your lab results, download your referrals, pay your bills and more.

Do not use Erie MyChart for medical emergencies. If you are having a medical emergency, dial 911 or go to the nearest hospital emergency room.

After Hours
If you are an Erie patient and you are feeling sick, you can call 312.666.3494 or 847.666.3494. An on-call provider is available when you are sick, even when the clinic is closed. Erie can help you with colds/flu, most fevers, sore throats and more. We can give you advice on how to feel better and we can help you decide if you need to see a doctor. The hospital Emergency Room is for very serious problems. If you are not sure, call Erie and we can help you decide.

Requests must align with and support the promotion of access to health, health education, and foster community engagement. Generally, a request must meet the following guidelines:

  • 60-day notice for health fairs, cultural events, and festivals
  • 90-day notice for sponsorships/health awareness campaigns

Note: Consideration may be given in special circumstances for events with shorter notice.
