As a patient of Erie Family Health Centers, the best way to review or copy your medical records for your personal use or for obtaining care from another healthcare provider is by using your Erie MyChart account at myeriehealth.com.
Simply sign in and click on the medical history page. You can print this information.
If you do not have a myeriehealth.com account, you can learn how to open an account by talking to a front desk representative at any Erie site, sending an email to our Help Desk, or clicking to learn more.
You can also receive a copy of your medical records for your personal use or for obtaining care from another healthcare provider.
You can also receive a copy of your medical records by downloading and filling out a copy of the necessary form.
