The mission of the Department of Community Engagement is to support Erie Family Health Centers’ vision of all people living their healthiest lives by addressing the barriers to health equity such as poverty, lack of insurance, and language and cultural differences.
We strive to achieve this mission by:

Community Needs Assessment
Erie Family Health Centers conducts a Community Needs Assessment (CNA) every three years, in alignment with health center program requirements from the Health Resources and Services Administration (HRSA). The assessment is designed to identify both the strengths and pressing needs within Erie's service area.
As required by HRSA, the CNA examines:
- Factors affecting access to and utilization of healthcare services
- The most significant causes of morbidity and mortality
- Additional social, economic, and environmental factors influencing health outcomes
Given that Erie's service area spans more than 84 ZIP codes, it was divided into six regions to facilitate efficient data collection and analysis. ZIP codes and communities identified as outliers within their respective regions were specifically highlighted throughout the assessment for deeper insight.
Following a thorough review of the 2025 Community Needs Assessment and its findings, Erie has identified four priority areas to guide efforts over the next three years. These focus areas will be addressed through Exemplerie — Erie’s FY25–FY27 Strategic Plan.
