Centered in the heart of Chicago’s West Town and Humboldt Park communities, Erie Division Street Health Center has been serving our neighbors for more than 50 years.
Erie Division Street Health Center provides a wide variety of services for adults and children, including primary care, reproductive healthcare, prenatal care, pediatric care and behavioral healthcare.
