Get Ready for Back to School

Erie Family Health Centers can help get your child ready to start the 2026 – 2027 school year.

Beat the end-of-summer rush and make an appointment for your child’s required school physical or dental exam today! Call 312.666.3494.

Students entering Pre-K, Kindergarten, 6th grade and 9th grade, and new students transferring from out of state, need a medical exam.

Students who do not have proof of this exam by October 14, 2026 will not be able to attend school.

  • Children entering kindergarten, 6th, or 9th grade don’t need a new school physical exam if they had a COMPLETE well-child visit after October 15, 2025.
  • Children in kindergarten, 2nd, 6th, or 9th grade need proof of a dental exam before May 14, 2027.
  • If you think your child had a complete well-child visit or dental exam and just needs the school physical form or school dental exam form filled out, message your Erie medical or dental provider in Erie MyChart, or call 312-666-3494 and leave a message with your child’s name, date of birth, and phone number.  We will review the request and either send the completed form within 10 business days or contact you to schedule an in-person visit if one is needed.
  • When you call us, please let us know if your child also needs an exam for their sports activities.  That may require an in-person visit, even if he/she had a physical since October.
  • Be sure to talk to your provider about any vaccinations that may be needed.

NO: Not this year.
Kindergarten YES: by October 14, 2026.
Exam must be completed between October 15, 2025 and October 14, 2026.
YES: before May 15, 2027.
Exam must be completed between November 15, 2025 and May 14, 2027.
2nd Grade NO: not this year. YES: before May 14, 2027.
Exam must be completed between November 15, 2025 and May 14, 2027.
6th Grade YES: by October 14, 2026.
Exam must be completed between October 15, 2025 and October 14, 2026.
YES: before May 15, 2027.
Exam must be completed between November 15, 2025 and May 14, 2027.
9th Grade YES: by October 14, 2026.
Exam must be completed between October 15, 2025 and October 14, 2026.
YES: before May 14, 2027.
Exam must be completed between November 15, 2025 and May 14, 2027.
12th Grade Special note: Students must have received one dose of the meningitis vaccine on or after their 16th birthday to enter the 12th grade. NO: Not this year.
Any grade if you are transferring from outside of the state of Illinois YES: by October 14, 2026.
Exam must be completed between October 15, 2025 and October 14, 2026.
