If you have not yet registered for an Erie Staff Vaccine Fair, please sign up below! The Staff Fairs will be held in the clinics at Erie Division Street, Erie Foster Avenue, Erie Evanston/Skokie and Erie HealthReach Waukegan.
Once you have been vaccinated, please fill out this form to attest to your vaccinated status. The deadline for staff to be vaccinated and to report it to Employee Health is OCTOBER 31.
What to Bring
- If you have insurance, you must bring your insurance card. (Uninsured staff will have their vaccines fully covered by Erie.)
- If you are eligible for a COVID-19 booster (see details below) please bring your CDC COVID vaccination card.
What Is Being Provided
- Flu Vaccine
- Tdap Vaccines
- Walgreens will also provide Tdap vaccines to those employees who need them to complete pending or expiring employee health requirements.
- COVID-19 Boosters
- Update Bivalent Booster will be available to staff who are eligible
- Eligibility includes: completion of ANY COVID-19 primary series, or previous booster shot greater than 2 months before the fair.
- IMPORTANT TO KNOW: Staff who are attending one of Erie’s vaccine fairs must take and complete an Erie-provided “vaccine passport” on the day of the fair. This will be the only available documentation for proof of vaccination. Each staff member is responsible for providing the vaccine passport to their care provider or one of Erie’s hospital partners. Staff who do not take a vaccine passport must reach out to Walgreens to retrieve vaccine documentation. Erie Employee Health will not keep copies of flu vaccines administered by Walgreens.

Flu Vaccine Facts
- Flu vaccines cannot give you the flu. Flu vaccines are made with either inactivated (killed) virus or with a single protein from the flu virus.
- I need the flu shot every year.
The Centers for Disease Control (CDC) recommends a yearly flu vaccine for everyone 6 months of age and older. Flu viruses change constantly, so the vaccine is reviewed each year and updated as needed, based on which flu viruses are making people sick. - We all still need a flu vaccine even though we are masked!
The flu shot is an Erie requirement and serves as another layer of protection from respiratory illness amidst the COVID-19 pandemic. It is the best way to protect yourself, friends and family, and our patients from the flu – which typically kills 12,000-60,000 people a year in the US.
