What to Bring to the Fair and What to Take Away
If you have insurance, you must bring your insurance card. (Uninsured staff will have their vaccines fully covered at these fairs.)
IMPORTANT TO KNOW: Staff who are attending one of Erie’s vaccine fairs must complete a provided Vaccination Record on the day of the fair. This will be the only available documentation for proof of vaccine administration.
- Each staff member is responsible for providing the vaccination record to their own care provider or one of Erie’s hospital partners.
- Staff who do not take the Vaccination Record will have to reach out directly to Walgreens to retrieve a proof of vaccine administration document which needs to include:
- your name and date of birth
- the vaccine name and lot #
- who administered your vaccine
- the date your vaccine was administered
Declination Form
If you choose to decline flu/Covid vaccine this year you must complete this declination form. It is required to EITHER receive the shots or sign the declination form.
What Vaccines Are Available
- Flu Vaccine
- The 2024-2025 Flu Vaccine Formulation
- The 2024-2025 Flu Vaccine Formulation
- Tdap Vaccine
- Walgreens will also provide Tdap vaccines to those employees who need them to complete pending or expiring employee health requirements.
- Updated COVID-19 Vaccine
- Update COVID-19 Vaccines will be available to staff who are eligible
- Eligibility includes any person (regardless of previous vaccination) if it has been at least 2 months since your last COVID-19 Vaccine dose.
Additional Details
Staff who are attending one of Erie’s vaccine fairs must take and complete a provided Vaccination Record on the day of the fair. This will be the only available documentation for proof of vaccine administration.

We encourage our insured staff to get their flu, COVID-19, (and Tdap if needed), vaccines with their local pharmacy or care provider.

Vaccine Facts
- Flu vaccines cannot give you the flu. Flu vaccines are made with either inactivated (killed) virus or with a single protein from the flu virus.
- I need the flu shot every year.
The Centers for Disease Control (CDC) recommends a yearly flu vaccine for everyone 6 months of age and older. Flu viruses change constantly, so the vaccine is reviewed each year and updated as needed, based on which flu viruses are making people sick. - We all need a flu vaccine!
The flu shot is an Erie requirement and serves as another layer of protection from respiratory illness amidst the COVID-19 pandemic. It is the best way to protect yourself, friends and family, and our patients from the flu – which typically kills 12,000-60,000 people a year in the US. - Flu and COVID-19 vaccines can be taken at the same time
